New Client Paperwork
Click on the PDF logo below to view our New Client Forms. You can either:
1) Fill them out on your computer, save it, and email it back to us. (Ideally, we would have these 24 hrs before your visit).
2) Print, complete the forms, and bring them with you to your first training session.
To complete:
1) My Journal Checklist - check off any item that represents how you are feeling using the past week as your guide.
2) My Journal - Tracker - List the top 3 things you would like to see shift/change in your life along with their presentation, duration, intensity, and duration. See the example for how to do this.
3) New Patient Form - fill out your information.
4) Informed Consent - read and sign.
The purpose of the Journal Checklist and List of Shifts is to drill down and identify exactly what you are experiencing, what is bothersome, and what you would like to see change. This helps us (and you) monitor your progress as we work with you during training.
